Tuition

Monthly Tuition (Full-time) 1 student:			$385.00
Monthly Tuition (Full-time) 2+ students:			$346.50
Monthly Tuition (Part-time) 1 student:			$325.00
Monthly Tuition (Part-time) 2+ students			$292.50

FULL-TIME Enrollment = 4 - 5 days/week
PART-TIME Enrollment = 2 - 3 days/week

There is a 10% discount if you enroll more than 1 student into our program
Monthly Payments begin in August and end in April
All payments will be made through Paypal Invoice, sent every 20th of the month
NEW families are required to pay a one-time enrollment fee of $100

MEDIA RELEASE FORM

I hereby grant permission to the San Diego Yokohama Sister City Society Connect the World afterschool enrichment program to use photographs or videos of my child(ren) for promotional or educational purposes, including but not limited to the program's website, social media, newsletters, and other marketing materials.

I understand that the use of my child(ren)'s image and/or voice is voluntary and that I will receive no compensation for the use of these materials. I also understand that my child(ren)'s image and/or voice may be seen or heard by the public, and that San Diego Yokohama Sister City Society Connect the World will make reasonable efforts to ensure that the materials are used in a responsible and appropriate manner.

I hereby release and hold harmless San Diego Yokohama Sister City Society, the Connect the World afterschool enrichment program, and their agents, employees, officers, directors, and representatives from any and all claims, damages, or liabilities arising out of or in connection with the use of my child(ren)'s image and/or voice.

I have read and understand this Media Release Form, and I agree to its terms.

EDUCATION CONSENT FORM

I give my consent for my child(ren) to participate in the San Diego Yokohama Sister City Society Connect the World afterschool enrichment program. I understand that this program includes educational activities and lessons, as well as field trips and other outings that may take place outside of the program's regular meeting location.

I also give permission to the Connect the World afterschool enrichment program to discuss my child(ren)'s educational records and performance with their teachers and school administrators, as necessary, for the purposes of evaluating and improving their educational experience. I understand that this information will be kept confidential, and will only be used for legitimate educational purposes.

I understand that my child(ren)'s participation in the program is voluntary, and that I may withdraw my consent at any time by notifying the program coordinator in writing. I also understand that my child(ren)'s participation in certain activities or outings may be subject to additional permission slips and waivers, which I will be notified of in advance.

I acknowledge that the San Diego Yokohama Sister City Society Connect the World afterschool enrichment program takes reasonable precautions to ensure the safety of all participants, but I also understand that accidents and injuries may occur. Therefore, I hereby release and hold harmless the San Diego Yokohama Sister City Society, the Connect the World afterschool enrichment program, their agents, employees, officers, directors, and representatives from any and all claims, damages, or liabilities arising out of or in connection with my child(ren)'s participation in the program.
